How much does home care cost?

If you have Medicare and your doctor decides you need home health care, you may choose any Medicare-certified agency that services your area with 100% coverage. We also accept other forms of insurance, which also require a doctor’s orders. You have a say in which agency you use. Choose Unique Health Care for compassionate and experienced help setup uniquely for you!


How does home health care work?

A care manager from Unique Health Care Solutions will come to your home for an assessment of your needs and medical condition. We will work with you and your doctor to set up an individualized plan of care, including all the services, treatments and equipment that you will need to reach the goal you set with the doctor. Your care manager, (your nurse or therapist) will act as a bridge between your physician and home. They will coordinate care with your entire care team and keep communication going to make the process as comfortable and quick as possible. When you have met the established goals, and your doctor feels it is appropriate, and/or you are no longer eligible, your home health services will be discharged. Your doctor may want you to continue with treatment at an outpatient facility or with regular follow up appointments we can offer Private Duty Services.


What qualifications does your team have?

Our first qualifications are compassion and experience. The other requirements vary for different positions. Nurses and Therapists have to be currently registered and certified in their fields. Non-skilled care can be provided by employees with sufficient experience or certifications to give you the highest quality care. Some hold the CTMA or CMA certificates. For your safety, Unique Health Care maintains worker’s compensation coverage and liability insurance. In addition, all of our employees are carefully screened and CPR certified. You can rest assured, you will receive safe, reliable, premium level care from a care giver you can trust and who truly understands your needs and concerns.
